Commercial Cinematography

If a client hires Evolving Media & Design for video services, the following Commercial Release form for conducting Video on location must be signed by you the client. This form lays out what is expected from all parties.

Model Release Form

If you intend to have people within your campaign, whether in print, Social Media, or the Internet. Each person who is visible, whether in a photograph or a video, must have a Model Release Form signed. A copy of this model release form is to be submitted to legal@evolvingmedia.com. A second copy is to be given to the model themselves, a third copy is to be retained for your records. If a person does not want to sign a model release form, they can not be used within the campaign, regardless if they are an employee at the time. Property Release Form

The following Form is required to be signed and provided if photography is requested on location of a rented retail business location or if the owner/ manager can not be present during the photo session. If you are the owner of the building or of the business location, then you do not need to fill this form out.
